GatorFest! Waiver

All students are required to sign the waiver to participate. If you are under the age of 18, your parents or legal guardians are required to sign. We use DocuSign to facilitate waiver signing. Please keep your email confirmation after you complete the signing. All students will be required to show proof they have completed the waiver prior to being granted admission to GatorFest! events.

Sign the Waiver (18+)
GF! Waiver for students 18 and over.

Instructions:

  1. Scan the QR code or click on the button above.
  2. Input your name and SFSU email address on the following page. Select: "Begin Signing"
  3. On the next page, click "Continue," then type your name and input your signature in the designated areas. Select "Finish"
  4. Download or print and save the form for your records.
Sign the Waiver (Under 18)
GF! Waiver for students 18 and under.

Instructions:

  1. Your guardian scans QR code or click on the button above.
  2. Your guardian Inputs their name and email + student's name and SFSU email address on the following page. Select: "Begin Signing"
  3. On the next page, your guardian clicks "Continue," then types their name and inputs their signature in the designated areas along with the student's name at the bottom. Select "Finish"
  4. Download or print and save the form for your records.
